Canada/Ottawa: Toys “R” Us Canada Ltd., a Concord, Ontario establishment, recalls an estimated 22,890 Addo Woodlets Lifting Puzzle Toys from the Canadian marketplace due to suspected component detachment and consequential risks of choking, oral injury, internal ingestion, respiratory failure. GI tract laceration damage, bowel obstruction and/or sepsis, all serious health and safety hazards requiring immediate medical intervention to minimize suffering and prevent life-changing injury.

Health Canada/Santé Canada (“HCSC”) reports this recall involves certain Alphabet, Vehicles, Safari and Farm Woodlets Lifting Puzzle Toys for children, as follows:

  • Addo Woodlets Lifting Puzzle – Alphabet, sold with a Toys “R” Us Canada SKU of 126009, an item/manufacturer’s code of 216117C and a UPC of 5056289402399
  • Addo Woodlets Lifting Puzzle – Vehicles, sold with a Toys “R” Us Canada SKU of 126017, an item/manufacturer’s code of 216122C and a UPC of 5056289404775
  • Addo Woodlets Lifting Puzzle – Safari, sold with a Toys “R” Us Canada SKU of 126020, an item/manufacturer’s code of 216121C and a UPC of 5056289404768
  • Addo Woodlets Lifting Puzzle – Farm, sold with a Toys “R” Us Canada SKU of 126023, an item/manufacturer’s code of 216120C and a UPC of 5056289404751

These Woodlets Lifting Puzzle Toys are thought to have been marketed, distributed and sold in all Canadian provinces although it is always possible these toy products had wider distribution outside the country. When in doubt, please contact the manufacturer, Addo Play Ltd. of Amersham, England, for specific product information.

According to Health Canada, the small plastic pegs used to lift the individual puzzle pieces from the base can detach from the metal screw holding the pegs in place. This defect can occur when utilizing the product in a reasonable manner expected with its intended design and anticipated use. If this defect occurs, the detached components could pose choking and other serious hazards to small children.

Proposed Remedy:
If you believe you have purchased or have in your possession any of the recalled Addo Woodlets Lifting Puzzle Toys, please do not use them nor let your child play with or near them. Instead, kindly return the product to the store where you purchased it for a full refund.
