US/Washington: Philips Lightolier Glass Lens Recall [US]Philips Lighting recalls approximately 7,500 Lightolier Glass Lenses due to laceration hazard. CPSC:

The US Consumer Product Safety Commission (“CPSC”) reports this recall involves Philips “Lightolier” brand AF4SY decorative Glass Lenses used on various types of light fixtures.

The clear glass lens, used to “soften” light with an additional lens layer, is 4-3/4 inches in diameter and 1/8 inch thick. The lenses were manufactured between January 2007 and December 2013. There are no markings on the product. The product number is on the packaging. Please consult photograph above for details of product design and presentation.

According to the CPSC, the decorative Glass Lens can shatter and result in falling pieces of glass, posing a risk of laceration to the consumer. Philips has received one report of the Glass Lens breaking. No injuries have been reported. Consumers should immediately remove the recalled Glass Lens and contact Philips Lighting for a free replacement unit.
