  • Coach and Equipment: Metro Lite Transit Buses and Phoenix Transit Buses equipped with certain Ricon S-series and K-series Wheelchair Lifts with an optional Armored Pendant Cable – failure of armored cable to clear wheel chair lift, contact with the power lug, electrical short circuit, fire hazards; potential number of units affected: 37
  • Federal Coach: Spirit and Premier Transit Buses equipped with A&M Systems-brand Door Actuators – voltage spikes may damage electrical components and circuits, actuator malfunction, overheating components, fire hazards; potential number of units affected: 30
  • HME: Ferrara Intruder Ember Cab and Chassis Emergency Vehicles equipped with IMMI-brand L9 Seat Belt Buckles – seat belt malfunction, unlatching difficulty, failure to conform to the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 209, “Seat Belt Assemblies”, emergency egress hinderance, injury hazards; potential number of units affected: 1
  • Nissan North America: Infiniti QX60, Nissan Pathfinder and Nissan Rogue Vehicles – failure to properly tighten wheel lug nuts, lug nut detachment, wheel separation, crash hazards; potential number of units affected: 334
