Additional information:
Health Canada/Santé Canada (“HCSC”) says this recall involves Ballast models DBAG-13P2C and SY-213HPF with a date code of “082003” and light fixture model LUNAR-2×13-120-HPF, with two plug-in 2-pin 13W compact twin tube fluorescent lamps.
The ballasts were sold separately to be installed into fluorescent light fixtures. Also, an unknown number of the same ballasts were used in production of the light fixtures mentioned above and such ballasts are built-in inside the light fixtures. The listed ballasts and the light fixtures are being recalled.
According the health Canada, high ambient temperatures might cause the recalled Ballast to overheat, creating a fire hazard. Symban Lighting has received two reported incidents of overheating.
